Payroll Year End Services Proposal for

Deliverables
In the course of this contract, Caselle will deliver the following:
W-2 Year-end Services
• Convert Employee information in Payroll to create W-2's and year end reports
• Copies of all reports used to create W-2s and W-3
• Provide applicable W-2 forms to customer to distribute to employees
• Provide file to upload to the Social Security website
• Update new federal and state rates in Payroll system
• Return updated GrO database with all W-2 related forms
1099 Year-end Services
• Convert Vendor information in Accounts Payable to create 1099's and year end reports
• Copies of all reports used to create 1099s and 1096
• Provide applicable 1099 forms to customer to distribute to Vendors
• Provide file to upload to the government
• Return updated GrO database with all 1099 related forms
Cost Requirements
The proposed agreement with Eagle includes services for Payroll year-end.
❑ Item 1: Provide W-2's and government files at $1,250
O Item 2: Provide 1099's and government files at$800
Total costs for payroll year end will not exceed $2,050.
**Eagle agrees to review transactions and reports at the end to verify that the terms and conditions of
this service proposal are fulfilled. Following verification any changes, corrections, or updates that need
to be made to data will be at an additional fee that is not included in the annual maximum.
