Service Agreement
THIS AGREEMENT is between Western Records Destruction, Inc., an Idaho Corporation, hereby known as the
"provider" and the customer listed below hereby known as"customer", for the purpose of the destruction
and disposition of confidential material.

CONDITIONS AND STIPULATIONS
1. The provider will pickup and destroy the confidential material in such a manner as to preclude any
subsequent extraction of information contained therein. Destruction will be conducted on-site.
2. All material identified for destruction will be clearly marked by customer. Destruction notices are available
by request.
3. Provider agrees to provide the above number of locking containers.
4. Customer agrees to return all containers listed above at the end of agreement.
5. Payment Terms: Net 30 Days
This agreement is for a period of One Year
commencing on 12/14/2023 and ending on 12/14/2024
