Payroll Year End Services Proposal for

Deliverables
In the course of this contract, Caselle will deliver the following:
ACA Service
• Convert Employee information in Human Resources to create ACA ready data
• Copies of all reports used to create ACA forms
• Provide applicable ACA forms to customer to distribute to employees
• Provide XML file to upload to IRS AIR system.
• Return updated GrO database with all ACA related forms and information for download.
Cost Requirements
The proposed agreement with Eagle, ID includes services for ACA.
❑ Item 1: Verify all required information into Human Resources module to complete ACA filing
requirements at $10 per employee for a maximum of $510.
❑ Item 2: Prepare all ACA forms and electronic files at $950
Total costs for payroll year end will not exceed $1,460.
**Eagle, ID agrees to review transactions and reports at the end to verify that the terms and conditions of
this service proposal are fulfilled. Following verification any changes, corrections, or updates that need
to be made to data will be at an additional fee that is not included in the annual maximum reported on the
prior page.
