ADDENDUM TO SERVICE AGREEMENT
COMMODITY OR SERVICE: Sanitga Supplies
PERIOD OF SERVICE AGREEMENT: October 1. 2018 — Sentember 30.2019
This Amended Service Agreement is entered into between the City of Eagle (herein called "City") and
Waxie Sanitary Supply, (herein called "Vendor'), for the purpose of extending the service agreement for
sanitary supplies.
POSSIBLE EXTENSIONS OF TERM OF AGREEMENT The parties may mutually agree to extend the
term of this Agreement. Such renewal will be in the form of an Amendment to the Service Agreement.
This addendum shall incorporate all conditions and requirements as stated in the original Agreement
between the City of Eagle and Waxie Sanitary Supply dated August 28, 2017 and ,hall remain in full
force and effect.
