POSSIBLE RENEWAL EXTENSIONS TO: September 30, 2019
Renewal: If this Service Agreement is subject to renewal into its second and or third years, such
renewal will be in the form of an Amendment to the Service Agreement.
This Service Agreement is entered into between the City of Eagle (herein called "City") and Waxie
Sanitary Supply. (herein called "Vendor").
Whereas, the City issued a Request for Quote (RFQ) dated October 19, 2016; and
Whereas, the City awarded a contract on November 1, 2016 pursuant to that process; and
Whereas, the City wishes to renew the agreement and Waxie Sanitary Supply wishes to continue to
provide products and services as request in the RFQ.
NOW, THEREFORE, in consideration of the mutual promises, covenants, and agreements hereinafter set
forth, Waxie Sanitary Sun Iv and the City of Eagle do hereby agree as follows:
1. Waxie Sanitary Supply agrees to supply such commodity or services in accordance with the
terms, conditions, and specifications in the Request for Quote (RFQ), including appendices and
amendments, and Official Bidding Form Response. The Request for Quote (RFQ) and the
Vendors response are incorporated and made a part of this Service Agreement.
2. The City and the Vendor further wish to clarify the following additional requirements, if any:

• The specific terms and conditions stated herein.
• Original Request for Quote (RFQ), including appendices/amendments.
• Contractor's response to the Request for Quote (RFQ)
• Official Purchase Order (when applicable).
4. This Service Agreement may be amended only by written agreement of the parties.

Request for Quotes
Sanitary Supplies
I. Purpose. This is a request for quotes for sanitary paper products for the
City of Eagle. Through this request for quotes, the City of Eagle is asking
vendors for proposals to supply products used in city facilities including
Park restrooms. The City of Eagle may offer the successful proposer two
(2) additional one-year renewal terms. The agreement term will run from
the day of award approximately November 1, 2016 through September
30, 2017, provided, however, that either party to the agreement shall have
the option to cancel and terminate the agreement by giving the other
party thirty (30) days written notice to the other party of its intent to
terminate.
II. Selection. The city of Eagle will select what in its sole judgement it
determines to be the quote that will be in the best interest of the City of
Eagle. Dispensing of some products will require the vendor to replace
proprietary despensors.
III. Delivery charges, if any, should be included in the quote.
IV. For&hat for quotes. Proposers may use the attached product sheet for
quotations. The format of the sheet includes an estimate on the quantity
of products needed.
V. Submission of quotes. Submit quotes to the City of Eagle by Friday,
October 28, 2016 by 5:00 p.m. local time.
