Dear Doug,
The City of Eagle has completed its review of the submitted quotes for "Sanitary Supplies". After review and
careful consideration, we are pleased to inform you that your company was selected. On behalf of the City, we
want to thank you for the time and effort in submitting a quote and we look forward to doing business with your
company regarding our purchase of sanitary supplies.
This letter will serve as the official agreement between Waxie Sanitary Supply and the City of Eagle for the. term
of November 1, 2016 through September 30, 2017, provided however, that either party to the agreement shall
have the option to cancel and terminate the agreement by -giving -the other parry thirty (30) days written notice to
the other party of its intent to terminate. The City of Eagle may offer two (2) additional one-year terms. Your
quote should include delivery charges. Please note the City of Eagle is tax exempt, you will find attached an
updated ST101 exemption certificate for your files. Authorized purchasers for the City will be Tina Bailey or
Brent Cluff with K & T Maintenance or Dave Rioux and Ellen Smith with the City of Eagle. If there are changes
in authorized purchasers, you will be notified by the City of Eagle. If you agree with the above terms please sign
below and return a signed copy for our files, as soon as possible.
