Notice: This contract was approved with an approved scope of work and a negotiated fixed Price Total of$24,075.00. The Eagle City
Council did not approve contingencies to this contact, all changes in program, materials, timeline and cost must be approved by the
Eagle City Council. Staff does not have authority to approve changes. All Change order requests must be submitted in writing including
this form for approval by the City of Eagle. Eagle City Council meets the 2nd and 4th Tuesdays of each month,change orders must be
received no later than the Wednesday prior to the meeting.
The contract documents are modified as follows upon execution of this change order:
1) Request to extend the project timeline through September 30, 2023 to allow the consulatnat to complete
the construction adminstation work with ACHD as detailed int he orginal task order.
2) Expand the scope of work to include: The design and prepare construction drawing for the inclusion in the
ACHD plan set for construction in 2023 of 3-2" and 1-3" conduit sleeves for City utilities including but not
limited to fiber optics within the paver section of the sidewalk. The design work should include both the north
and south side of Eagle Road running east/west with the terminus of the project consistent with the proposed
ACHD sidewalk improvements or another logical boundary. The design work should also include stubs at all
road crossings and junction boxes at the shared corners of all property lines. The project should include an
estimate for construction and post construction survey of the exact location of the conduit,junction boxes
and stubs.
