Authorization for Additional Services CLIENT: City of Eagle Project Name: Eaale Island/Mace Pathway J-U-B Project Number: 10-18-113 1. Additional Services. The following additional items of work on the project referenced above have been or will be provided by J-U-B ENGINEERS, Inc.(J-U-B). These Additional Services are a supplement to the scope of services contained in J-U-B's existing Agreement for Professional Services for this Project,dated October 22,2018. All other TERMS AND CONDITIONS of said Agreement remain in full force and effect. Task 006-Newman Prooertv Boundary Adjustment and Record of Survey(Ada County): J-U-B will oerform the necessary field work and office support to process a orooertv boundary adjustment in Ada County between the Newman Property(Tax ID 50518131350)and the Idaho Department of Lands property adjacent to and easterly of the northerly portion of the aforesaid Newman Prooertv. New orooertv lines will be as provided in the email from Steve Noyes. with the City of Eagle dated 1/13/2020(Attachment"A"). , J-U-B is responsible for the following: -Perform boundary and location surveys to collect sufficient data to process the Property Boundary Adjustment. -Complete and submit the application to Ada County -Uoon approval. furnish all necessary legal descriptions to the client and the title company -Set appropriate survey monuments marking the newly adjusted lines and file a record of survey with Ada County, The City of Eagle is responsible for the followino: -Obtain reauired sionatures by Prooertv owner on applicable documents. -Provide title reports. necessary documentation, and fees.if reauired(apolication and recording fees) needed to process the application. Assumes new parcel size and confiouration is acceptable to Ada County(in Substantial Conformance per ACC 8-1A-11. Task 007-Newman Prooertv Boundary Adjustment and Record of Survey(City of Eagle). Task 008-IDL Disclaimer Application and Survey for Newman Property J-U-B will assist with oreparing the application for disclaimer with IDL(the City of Eaole will obtain necessary signatures and, IDL fees for the application) boon IDL acceotance of apolication.J-U-B will Perform the necessary fieldwork to locate the. areas identified by IDL as the old OHWM(Item 3 of IDL Disclaimer procedure)_ In conjunction with Task 007 above.J-U-B will conduct the necessary office work and fieldwork to prepare and file a record of survey and Prepare a legal description to be. used for the disclaimer document. J-U-B is responsible for the following: -Assist with oreoarina IDL Disclaimer Apolication. -Perform necessary fieldwork to locate areas identified by IDL as the old OHWM. Task009-Leaal Description for Pathway Easement J-U-B will oerform the necessary work to prepare a legal del1 criotion for the City of Eagle's use in recordina a oublic Pathway easement along the proposed trail route across the lands of IDL(as adjusted in the previous tasks). The easement will be 20- feet wide and centered on the pathway alignment(width will be areater if needed at bridge location). 2 [This#2 intentionally left blank] 3- Payment for Additional Services. Unless otherwise noted below, J-U-B will provide these Additional Services on a time and materials basis, using J-U-B's standard billing rates or,if applicable,the billing rates established in the initial Agreement for Professional Services. Other Basis for Payment: Payment will be made on a lumo sum basis in the following amounts: Task 006:$7.220 Task 007:$9,660 Task 008. $8.570 Task 009: 1 980 Total: $27.430 4. Schedule of Services. Due to the Additional Services,the Schedule of Services to be performed under the original Agreement for Professional Services is modified as follows: Additional services will be provided within three(3)months of the execution of this supplemental agreement and weather oermittina.
