Description of Additional Services Reauired:
Amend the current contracted scope of work to include additional services necessary to prepare agency
approval applications, retain geotechnical engineering services and perform Construction Documents,
Bidding and Construction Administration Services for the Phase I scope of work as outlined within the
letter to City of Eagle, dated July 15, 2004.
The terms and conditions of the original Agreement dated April 8, 2003, shall apply to the services
provided in this Authorization for Additional Services. This authorization for additional services shall be
considered an addendum to the original contract with the Architect/Engineer.
Compensation shall be the total sum of: Fixed Fee of $86,637.00, plus
Drintina expenses
This Authorization for Additional Services constitutes your acceptance and authorization for the Architect
and/or Engi eer to proceed with the wor described above.

Dear Mayor Merrill:
We are pleased to see the continuation of the Hill Road project progress from concept to construction
documents. As stated in our meeting on July 8, 2004, the city of Eagle has set aside funds for the
construction of the park. We have provided a revised format outlining the moneys available. We are
prepared to move forward quickly in an effort to get this project underway before winter weather sets in.
We see this effort as an amendment to our existing contract with the city of Eagle (dated April 8,2003).
The proposed scope of work will be prepared under the terms and conditions of the existing contract.
This letter simply summarizes a scope of work for CSHQA and identifies the fund allocations as they
relate to the construction scope of work. CSHQA will perform the following:
I. Prepare full Design Review Board application-package, represent the city of Eagle for approval
of the overall master plan and gain site-specific approval for the scope of work outlined as Phase
1. (See attached Appendix 'A'). Our intended Design Review submittal date is August 5, 2004.
2. Retain the services of a licensed geotechnical engineer to perform subsurface investigations of the
park site to establish soil porosity, paving and structural needs of the master plan. Three copies
of the report will be provided to the City for records and the report will be certified to both
CSHQA and the city of Eagle.
3. Upon approval of the Design Review Application CSHQA will prepare construction documents
(plans and specifications) that represent both the base bid and alternate scopes of work identified
in the attached spreadsheet (Appendix 'B'). This spreadsheet was prepared utilizing funds
available under Fiscal Year 03/04 totaling $404,825.00; projected funds for Fiscal Year 04/05 are
$435,685.00. (See attached Appendix 'c' provided to CSHQA at the July 8, 2004 meeting).
a. Note: The city of Eagle may elect to authorize CSHQA to proceed with construction
documents prior to Design Review Board approval. If changes are required of the
construction documents as necessitated by the Design Review Boards site 5pecific or
standard conditions of approval, the documentation effort would be performed as an
additional service to this contract.
4. Continue to assist the city of Eagle with public bidding, contract award recommendations and
construction administration, per the conditions of the established agreement.

Our proposed fee amendment for the work described above would be Eighty Six Thousand Six Hundred
Thirty Seven and no/IOO Dollars ($86,637.00). This fee includes labor and expenses (including typical
out-of-pocket expenses (travel, mileage, reproduction of plans and presentations)) required, but does not
include expenses associated with the printing costs of the contract documents for bidding distribution.
CSHQA offers competitive printing prices for our clients and would commit to a printing cost of:
. 30" by 42" sheet $1.53 per copy
. 8-1/2" x II" sheet $0.05 per copy
Additional Services:
The services listed below are not included in our proposed scope of work. If requested by the Owner, the
Additional Services listed below will be provided on a Time and Material basis. The Owner will be
notified in a timely manner of the status of an additional service issue before proceeding with the work.
Architectural and Landscape:
. Value engineering, or the cost benefit analysis of any system or construction method
previously directed by the Owner to implement.
· Owner requested changes to the documents or construction systems once partially or fully
implemented will be an additional expense.
· Three-dimensional computer generated renderings or streetscape perspectives, if required by
the Regulatory Agency or if desired by the Owner.
Civil:
· Revisions resulting from changes in the site plan once the final civil design has begun will be
considered additional services.
